It covers the period between ages 14 and 25.<\/p>\n\n\n\n<p>It may include steps to help you:<\/p>\n\n\n\n<ul class=\"wp-block-list is-style-primary-bullets\">\n<li>Move to adult healthcare services.<\/li>\n\n\n\n<li>Move to adult social care services.<\/li>\n\n\n\n<li>Go from school to college.<\/li>\n\n\n\n<li>Get a job or start training.&nbsp;<\/li>\n\n\n\n<li>Manage housing and benefits.<\/li>\n\n\n\n<li>Prepare generally for adult life, including managing money, transport, a social life, friendships and relationships.<\/li>\n<\/ul>\n\n\n\n<p>The most important thing is to start thinking and planning early and get support to help you through your transition process.<\/p>\n\n\n\n<p>Here you will find information to help you do
